Responsibilities

  • The RN Case Manager assesses the social, psychological, cultural, environmental and financial situations concerning
  • the patient and family.
  • He/she is a patient/family advocate, promoting rights and dignity and striving to involve the patient/family in aspects of care as appropriate.
  • he RN Case Manager also facilitates the acute, rehabilitative, and long-term discharge processes to provide for an optimal age-specific continuum of care for the populations listed below.
  • Accountable for the care of patients assigned to them in accordance with the Nurse Practice Act and Policies and Procedures of BSA.

Qualifications

Job Requirements:

  • Graduate of Accredited School of Nursing
  • Current License as RN in State of Texas

  • Basic Life Support Certification

  • Position requires fluency in English language to function in job environment appropriately

Preferred Job Requirements:

  • Registered Nurse with two years of experience, preferable in a hospital setting
