As a Community Services Case Manager (CSCM) in our Patient Support Services (PSS) team, the incumbent's primary role is to play an essential part in this dedicated team, utilizing clinical expertise, effective communication, problem-solving skills, and conflict resolution techniques. The position places a strong emphasis on robust assessment skills, exceptional organizational abilities, and efficient time management. Case management, at its core, is a collaborative process that involves assessing, planning, implementing, coordinating, monitoring, and evaluating patient options and services to meet their unique healthcare needs through effective communication and the utilization of available resources.

Minimum Qualifications

  • Valid Driver's license.
  • Two (2) years' of experience in case management, including coordinating care for patients with complex psychosocial issues.
  • Bachelor's degree in social work, human services, or related field.
  • Knowledge of individual, child, and family development, human behavior, communication, cultural influences, social work practice, psychotherapy, case management, and the integration of social work with medicine.
  • Demonstrated knowledge of child, elder abuse, and domestic violence reporting requirements, as well as relevant legislation (e.g., Tarasoff, patient confidentiality).
  • Demonstrated problem-solving skills to resolve differences in a cooperative spirit, create mutually beneficial solutions, and effectively communicate within the clinic team, other UCDHS departments, external associates, and patients.
  • Demonstrated skills and experience in utilizing community resources.
  • Demonstrated ability to conceptualize and interpret clinical situations, applying theory in case management and psychotherapeutic responsibilities.
  • Demonstrated experience with self-directed, patient care management. Knowledge of health care, healthcare delivery systems and managed care.
  • Demonstrated knowledge and experience interviewing and communicating, to elicit information and cooperation from patients/ clients.
  • Demonstrated knowledge and experience with establishing and maintaining effective working relationships with patients/clients, staff, and community resources.
  • Demonstrated knowledge and experience writing reports, reflecting interventions and document progress.

The UCD Hemophilia Treatment Center (HTC) is a federally funded, registered treatment center that provides comprehensive medical, psychosocial, and specialized care to individuals with rare inherited bleeding and clotting disorders.
